By signing below, I hereby acknowledge that I fully understand and agree that I will at all times comply with these requirements.
STUDIO RULES INCLUDE: ALWAYS BRING A SWEAT TOWEL TO CLASS AND PARTICIPANTS ARE TO BE IN THE STUDIO BEFORE THE CLASS STARTS. THE DOOR IS CLOSED AS SOON AS THE CLASS BEGINS.
F45 Health & Fitness Liability Waiver
I, the participant named below, minimum 18 years of age, have agreed to participate in the F45 workouts, exercise and training programs (“Workouts”).
I acknowledge and agree that the Workouts:
- are a recreational sport activity; and
- may involve strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning and training, and other various fitness activities.
I hereby affirm, and I affirm each time I participate in a Workout, that:
- I am in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation in this exercise programme; and
- I am participating in the Workouts voluntarily and at my own risk.
I hereby release F45 Training Pty Ltd, F45 Training Inc, F45 Training Private Asia Ltd, Functional 45 Training Ltd, F45 Training Canada Ltd and their related body corporates, affiliates, successors, F45 Training Ealing and their officers, agents and staff (the “Released Parties”) from any claims, demands, and causes of action as a result of my voluntary participation in the Workouts, to the extent permitted by law.
I fully understand that I may injure myself as a result of my participation in the workouts and I hereby release the Released Parties from any liability now or in the future for conditions that I may obtain directly or indirectly from participating in the Workouts, to the fullest extent permitted by law. These conditions may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, injuries to knees, injuries to back, injuries to foot, or any other illness or soreness that I may incur, including death.
I will adhere to local government guidelines and ensure that I follow the reasonable directions of all F45 staff and trainers in relation to my participation in the F45 training sessions. I will diligently keep equipment and stations clean and I will sanitise my hands upon entering and exiting the studio. I have reviewed and understand the cancellation policy and waitlist process.
Cancellation Policy & Waitlist Process
The cancellation window is 8 hours for classes starting before 12 midday and 2 hours for classes from 12 midday. Should you cancel or not show for a class, fees are automatically processed and strictly applied. For a Limited Membership, drop-in class, class pack, there are no fees but the class is deducted from your account. Late Cancel fee is £5 & No Show is £15. These are processed the day after the class it relates to. This does not apply for Class Pass or WellHub members, they are managed directly by the partnership company.
If you are on a Waitlist you will be automatically moved & immediately confirmed on the class when a space becomes available.
